Contact Lens Prescriptions expire after 1 year in the State of California.
A Contact Lens Fit or Evaluation is required to renew contact lens prescriptions in the State of California. This includes up to 90 days for follow-up visits to change brands or power.
1) Please submit form. We'll email your balance before and after applying insurance allowance.
2) Pay online balance by Square invoice.
3) All orders will direct ship to address provided, unless requested to pick-up in office.
We cannot cancel direct ship orders.
We will try to respond within 1 business day.
Our office closes at 1pm on Saturdays and closed on Sundays and Mondays.
No Returns on contact lens boxes. These are medical devices and will not be resold.
Acuvue products will take up to 2 weeks to receive due to delays with the manufacturer.
