We may or may not accept: Cigna, Guardian, Metlife, United Health plans, depending if vision plan is VSP and may require special ID#.

We do NOT accept:  Avesis, Davis Vision, NVA, Superior Vision, VBA,

Medi-Cal VSP plans, Medi-Cal EyeMed plans, Medicare, Santa Clara Valley Health, CSU student EyeMed plans, Target-Only EyeMed plans

If you have a Medi-Cal VSP plan, we are not in-network. To find a Medi-Cal VSP provider, please visit VSP.com, "Search Providers" and go to "Advanced Search", select Doctor Network "Medicaid".
