This is optional if you want to give our team a heads-up, so we can have your glasses or insurance benefits ready when you arrive.
Our team will send you an Adobe PDF health history form to fill out online prior to your eye exam. Or you can download and print a copy. If unable to fill out online, patients can fill out in-office. This helps reduce wait time.
